Jason Allen Hopkins, 39, of Ledbetter died on Saturday March 28, 2015 at his residence.
Mr. Hopkins was a former nurse tech and a member of Ohio Valley Baptist Church.
Surviving are his parents, David Allen Hopkins and Jeannie Blankenship Hopkins; one brother, David Lynn Hopkins of Ledbetter.
Celebration of Life Service will be private for family and close friends. Burial will follow at Oak Grove Cemetery in Ledbetter.
Smith Funeral Chapel in Smithland is in charge of arrangements.
